Please select the appropriate form for your service, or ask us if you need help in determining which form to complete. If you do not fill out your forms prior to your appointment, please arrive 15 minutes early to do so in person, or there may be a delay in services or a shortened session. These forms are required by law and help protect both Cloud9 Spa and our wonderful clients, so we appreciate your cooperation. Some of our services are not compatible with some health issues, so please be honest when completing the forms.
